Created attachment 187 [details] Screenshot of the broken page As you can see in the attached screenshot, the layout of this page is completely broken. Labels for radio buttons are off, text inputs are not aligned and the "Modify" buttons are randomly placed.

Although some of the issues have been fixed, others have been introduced. Please check the attached screenshots.
